Preschool Application 2024-2025

I hereby request space for my child for the program time and option specified below. I understand that I am committing to a ten month academic school year program and tuition payment.

I have enclosed with my application the required 2024-2025 enrollment deposit for new and returning students to secure a place for my child (will be credited to June 2025 tuition balance).

Enrollment deposit is non-refundable after August 1, 2024. Application fee is non-refundable and does not apply towards tuition.

In order for the application to be complete, you must download, fill out and submit the following documents.

Immunization Form

Parents who choose to receive some or all vaccination exemptions, please click here to transfer to the State of Oregon Health Authority website for more information. Follow the "For Parents" link to complete the online module and receive your online vaccination education certificate. Certificates may be printed or a screen shot may be taken to upload below for enrollment
